What happens if the catalytic appliance is operated improperly?

Plugging can occur in the combustor if the stove is operated improperly. (closing the bypass to soon)  Always allow 25 to 30 minutes of burning before closing the bypass.
Doing this will also not allow the combustor to achieve light-off and do its job.
Another example is burning materials that produce large flakes or char, such as wrapping paper or cardboard.  This can plug enough cells to cause smoke spillage.
